The following steps will guide you through our enrolment process.
ENROLMENT FEE
An enrolment fee applies. The fee is currently set at $50 for the first child enrolled. Any additional children will incur a cost of $30 per child.
UP TO DATE IMMUNISATION
We require your child’s Immunisation History Statement showing your child’s immunisations are up to date to process your child’s application.
Other immunisation records, such as the Interim NSW Vaccination Objection Form, Blue Book, a GP letter or an overseas immunisation record are not acceptable.
MEDICAL ACTION PLANS
If your child suffers from a chronic illness or medical condition such as asthma or anaphylaxis, an action plan is required to start care. These plans are valid for 12 months from the date of issue and must be current upon enrolling. Action plans must be prescribed by your child’s doctor or specialist.
CUSTODY/COURT ORDER DOCUMENTS
Five Star Family Day Care cannot deny access to parents listed on a birth certificate without a custody/court order.
BIRTH CERTIFICATE
Please note that we do not require the Birth Certificate to be uploaded before enrolment if you are unable to access it easily. You are able to email the document to the office or give a copy to your Educator at a later date.
